Canadian Pharmacy Calls for Safer Post-Op Drugs to Avoid Increased Infection

Post-operative care usually includes antibiotic regimes to fight infections acquired through hospitalization including C difficile. Researchers from UK have now found infections are likely to increase if new antibiotic regimes are introduced after particular surgical procedures. Patients undergoing surgical procedures for diagnosis of prostate cancer were found to have five times more infective complications with the use of a new standard antibiotic regime. Researchers from Addenbrooke’s Hospital in Cambridge found a number of cases resulted in sepsis and one case of septic shock.

The new regime of medications was introduced to monitor rate of hospital-acquired infection as well as post-operative infection. Changes initiated by regulatory bodies show that C difficile rates were accelerated because broad-spectrum antibiotics were being used on a large scale. Even though increase in C difficile cases were not indicated in hospital records, the number of infection cases after hospitalization increased to a level forcing regulatory bodies to ask hospitals revert back to the original standard practice used before the new regime was introduced.

The study included monitoring results of 709 consecutive patients undergoing post-operative care after receiving transrectal ultrasound-guided prostate biopsy surgery (TRUSP Bx). Post-operative care of these patients was monitored for a total period of 20 months.

Several indications were recorded including a significant finding of just 2.4% of 454 patients treated with the original regime of medications developing infection within four weeks of the surgery. Around 0.9% with infections had to be readmitted into hospital. Those treated with the new regime indicated a 12.9% infection rate within four weeks of surgery, out of which 4.7% had to be readmitted due to complications.

Patients on both regimes did not develop C difficile within 30 days, though patients who were reintroduced to the original regime demonstrated significantly lower risk of getting infections. In fact, none of the modern results indicated serious complications in patients put on the original regime, while seven patients readmitted to hospital suffered from sepsis. An additional case suffered from septic shock. All of these cases were on the new regime.

The study is first of its kind conducted on patients suffering from infective complications arriving out of surgery. Research was based on earlier documentation and comparison of different regimes. Findings also indicated a general medical regime cannot be followed for treatment of patients located in different places.

Prophylactic regimes have to be locally determined to conduct a procedure like TRUSP Bx to ensure post-operative infections are avoided. Complications could arise in other medical procedures as well, if systemic changes are made in post-operative care procedures without considering long-term effects. Documentation based on local results will help arrive at the right medical regime for post-operative care.
